Log on to your account or create a new account at www.mbpartnerships.com. 2. Click on Redeem a Certificate and enter your Certificate Number. 3. Only enter your certificate number once. The credits will appear in your account. Each credit is redeemable for one service, regardless of the face value of the service. 4. Browse our services and add the offers you like to your cart. 5. Please choose only one service from any category. 6. Continue through the checkout process. Do not enter your certificate number again. Your credits will be applied and your balance will be $0.00 once you confirm the order. 7. The services you selected will now be available under My Account. 8. Choose VIEW and click on the service you wish to print. 9. Services, once selected, have a 6-month recommended use date. Choosing to Have Your Services Mailed: If you choose the mailing option for gift certificates there is a shipping and handling fee of $5.95. The Mind Body Partnerships customized package of services will arrive at the shipping address you provided within 5 to 7 business days. Scheduling Appointments: Please wait till you have printed or received your service Call the specified service provider on each certificate directly to schedule appointments. Mention the certificates at the time of booking and bring your certificate with you at the time of your scheduled visit. Please Note: Once certificates have been redeemed by electronic delivery or mail there will be a $10 fee to exchange any certificate option. Please make your selections with care. Policy Governing Redeemed Service-Specific Certificates 1. Certificates are valid for new clients only for any particular service provider. 2. Only one certificate will be honored per person with any particular service provider. 3. Certificates must be presented at the time of service or the appointment scheduled will be charged at full price. 4. A minimum 24 hours cancellation notice is required to reschedule a service or visit; otherwise that particular service will be forfeited. 5. Certificate cannot be redeemed for cash, only for the services explicitly outlined on the certificate. 6. Certificates are not exchangeable for products. 7. Certificates are valid only for specified services and cannot be substituted with other services. The individual services described in this package are subject to change slightly, based on the ability of the facility to perform them. 8. Appointments are based on availability. 9. Please allow at least 2 weeks for appointments. 10. This offer cannot be combined with any other offer, coupon or discount program. 11. Mind Body Partnerships is an independent company and not affiliated with any service partner. 12. Certificates are transferable. 13. Neither Mind Body Partnerships nor any promotional partner nor any partner spa is responsible for loss, theft, or mutilation of this certificate. 14. Gift certificates are subject to an expiration date, if any, set forth on the face of the certificate, except where prohibited by applicable law. 15. The person redeeming the services must work or reside within 10 miles of the specified service unless otherwise stated on the certificate. Mind Body Partnerships service guarantee. All services are guaranteed for 6 months from the date of their issue. During the 6 month period Mind Body Partnerships will exchange your service for free under the following conditions: a. The service provider is unable to provide the service within 3 weeks of customer’s call. b. Customer is unsatisfied with the quality or professionalism of the service. c. The service provider does not have a reasonable availability in their schedule to accommodate the customer. Exchanges & Lost Certificate Replacement. Requests for exchanges after 6 months from the date of issue will require a $10 Processing Fee. Requests for exchanges when there is nothing wrong with the service, the customer changes their mind about the service will require a $10 Processing Fee. Requests for exchanges when the customer received it as a gift and wants a different service requires a $10 Processing Fee. Lost or stolen certificates can be replaced if the customer can provide a valid certificate number or the order can be traced back to the original purchaser. Each lost certificate replacement requires a $10 Processing Fee. Replacements and Exchanges must be paid for with a credit card over the phone. Flat–Fee Shipping & Handling Paper Certificates will be mailed using US Postal Service: 5–7 business days. A flat fee of $5.95 will be added to your order for each certificate. Payment Options We accept the following credit cards: Visa, Master Card Discover, and American Express. All Credit Card charges will appear as "Mind Body Partnerships". User Legal Terms Neither Mind Body Partnerships nor any promotional partner makes any warranty of any kind regarding any service you may receive in connection with your use and redemption of this certificate. Neither Mind Body Partnerships nor any promotional partner assume any responsibility and shall not be liable for any injury, loss, claim, cost, damage, or any special, exemplary, punitive, indirect, incidental, or consequential damages of any kind, whether based in contract, tort, strict liability, or otherwise which arises out of or is in any way connected with (i) your use and redemption of this certificate or (ii) the performance or non-performance by any third-party providers even if you have been advised of the possibility of damages. Under no circumstance shall Mind Body Partnerships or any promotional partner be liable for any delay or failure of performance resulting directly or indirectly from acts of nature, forces, or causes beyond its reasonable control.
